Requirements for Registration in Henrico County Public Schools
To ensure prompt service, we would like to encourage all families who are registering a new student to make an appointment with the registrar, Laura Mondrey by calling (804) 756-3010.
Grades 1 – 12 Enrollment:
First-Time or Transferring to a Henrico School
When you register your child, bring the following information with you:
- Certified Birth Certificate (must be a certified copy).
- Physical examination***
- Immunization Record
- Social Security Number (must be provided within 90 days of admission. If ineligible to obtain a social security number, please submit a written request to the Superintendent, who may waive this requirement.)
- Transcripts: For secondary school pupils, your new school will send for a transcript.
- Helpful, but not required: Withdrawal papers from last school attended.
- Copy of last report card.
- Proof of Residency (In the case of families living with other families please contact the school for Proof of Residency/ Affidavit forms and requirements.)
- House contract, lease agreement, rent receipt, mortgage statement, or tax assessment; AND Current utility bill; AND Picture ID: Driver’s license, DMV or Military ID
*** All students transferring to schools from other systems in Virginia or from out-of-state must meet certain state medical requirements, and documentation must be presented before the children may attend school.
The requirements include:
- A complete physical examination by a licensed physician, a licensed physician assistant or a licensed nurse practitioner acting under the supervision of a licensed physician performed no earlier than 12 months prior to the date such pupil enters school.
Elementary ONLY
- Successful immunization against Diphtheria, Tetanus, Whooping Cough, Polio, Rubella (German Measles), Red Measles, and Mumps. For children born on or after January 1, 1994, a minimum of three (3) doses of Hepatitis B vaccine is required.
- Additionally, the following vaccinations are required: Varicella vaccine (Chicken Pox) required for children born on or after January 1, 1997 and Hepatitis B vaccine (three completed doses) required for students entering the 6th grade as of July 1, 2001.
Other Considerations for Parents
- Only a natural parent, adoptive parent, court appointed legal custodian or a person with a military power of attorney may enroll a child. (Non-military or traditional powers of attorney are not acceptable.)
- A foster parent may enroll a child with the consent of the child’s legal custodian. Foster parents enrolling children in their care must present at enrollment a copy of the Court Order that identifies the child’s legal custodian.
- If your child currently receives Special Education Services, bring a copy of the current IEP or 504 Plan.
Address Change Procedure
A parent or guardian must present, in person, two (2) proofs of residency (lease, contract, deed, tax assessment AND utility bill). If a current utility bill is not available at the time of the address change, a contingency address change can be initiated. The utility bill must then be presented within 30 days after the date of the address change. A picture ID is required to verify the identity of the person making the address change request. Bus information for the new address will be provided upon receipt of proper documentation.
Transcript Request
Current Students
- Current students who need a copy of their transcript for any reason should fill out the “Transcript Request Form” and return to their counselor.
- Please allow 48 hours for transcript to be prepared
- Current students can receive 2 transcripts for FREE and each additional transcript is $3.00.
Former Students
- Former students who are living in the area can request a transcript in person using the “Transcript Request Form”.
- Please allow 48 hours for the transcript to be prepared.
- Former students who are living out of the area should request a transcript by calling 756-3010.
- The cost for a transcript is $3.00 and should be paid at the time of request.
**Students graduating before 2001, will need to obtain their transcript from HCPS Centrals Records located on Nine Mile Road. Please contact:
